CHIEF COMPLAINT (CC) "I've had this red rash on my stomach and back for the past three days, and it's starting to spread. It’s really itchy." 2. HISTORY OF PRESENT ILLNESS (HPI) Onset: The r... ash began approximately 3 days ago. It started on the lower abdomen as a few small, red bumps. Location: Initially on the abdomen, now has spread to the flanks, upper back, and proximal thighs. It is bilateral and symmetrical. Duration: Continuous since onset. No lesions have completely resolved, though new ones continue to appear. Characteristics: Described as intensely pruritic (itchy). The patient reports a "pins and needles" or mild burning sensation in the skin of the left lower back, which started about 24 hours before the rash appeared in that spot. Aggravating Factors: The itching is worse at night and after a hot shower. Relieving Factors: Over-the-counter 1% hydrocortisone cream provides minimal, temporary relief. Cool compresses help slightly. Timing: Constant itch, with intermittent sharp increases in severity. Severity: Rates the itching as 7/10 when at its worst. Associated Symptoms: Systemic: Denies any fever, chills, night sweats, or unexplained weight loss. Constitutional: Reports feeling slightly more tired than usual over the past week. ENT: Denies sore throat, cough, or rhinorrhea. GI: Denies nausea, vomiting, diarrhea, or abdominal pain. MSK: Denies joint pain or swelling. Neuro: Denies headache or photophobia. Recent Activities/Exposures: Travel: No recent travel outside of the state. New Medications: No new prescription medications. Denies use of antibiotics recently. New Foods: Reports trying a new brand of protein bars a week ago, but has eaten them several times since without issue. New Contacts: Started dating a new partner about 3 weeks ago. Partner has no known rashes. Animals: Has a cat at home that has been healthy. No new animals. Stress: Reports high stress levels at work over the last month. Sick Contacts: No known sick contacts at home or work. 3. PAST MEDICAL HISTORY (PMH) General: Generally healthy. Childhood Diseases: Varicella (chickenpox) as a child. Surgeries: None. Hospitalizations: None. 4. MEDICATIONS OTC: Multivitamin daily. Ibuprofen 400mg PO once or twice a month for headaches. PRN: Hydrocortisone cream 1% (used on rash for past 2 days with minimal effect). 5. ALLERGIES Drug Allergies: No Known Drug Allergies (NKDA). Environmental: Seasonal allergies (pollen) - causes sneezing and itchy eyes in the spring. Treated with OTC loratadine as needed. 6. FAMILY HISTORY Mother: Alive, age 55, history of hypothyroidism. Father: Alive, age 58, history of hypertension. Siblings: One younger brother, age 22, alive and well. Family Skin Conditions: Denies any history of psoriasis, eczema, or skin cancer in first degree relatives. Grandfather had shingles (herpes zoster) several years ago. [Show More]
