Mr. Washington’s symptoms of nausea, anxiety, diaphoresis, mild auditory hallucinations, and history and physical exam finding... s of chronic alcohol abuse, positive screening via AUDIT and Severity of Alcohol Dependence questionnaires, tremors, agitation, tachycardia, ataxic gait, impaired rapid alternating movements, mild hepatosplenomegaly, and decreased sensation make this the leading diagnosis (Pace, 2018; Muncie et al., 2013). Additionally, the onset of Mr. Washington’s withdrawal symptoms is consistent with the timing of acute withdrawal, as this occurs within 6 to 24 hours of the last consumption (Pace, 2018; Muncie et al., 2013). Laboratory findings of an elevated blood alcohol concentration, decreased magnesium and phosphorus, low-normal folate and thiamine, and elevated liver enzymes further support this as the leading diagnosis (Pace, 2018; Muncie et al., 2013). The finding of increased sulcal spaces on head imaging further supports this diagnosis, as this is an indication of chronic alcohol use (Pace, 2018). [Show More]
