Oxygenation: Ventilation & Perfusion
Brunner and Suddarth: Chapters 21, 22

ATI Pharmacology Review Module: Pages 111, 119
Objectives
- Describe ventilation, diffusion, perfusion, and ventilation–perfusion imbalances
- Identify the diagnostic tests used to evaluate respiratory function and related nursing implications.
- Describe the nursing management of patients receiving oxygen therapy, incentive spirometry, smallvolume nebulizer therapy, chest physiotherapy, and breathing retraining.
- Discuss the patient education and transitions in care considerations for patients receiving oxygen
therapy.
- Describe medical and nursing management of patients with select upper airway disorders (rhinitis,
pharyngitis, tonsillitis, peritonsillar abscess, epistaxis, obstructive sleep apnea).
- Identify patients at risk for atelectasis and the nursing interventions related to its prevention and
management.
- Compare the various pulmonary infections with regard to causes, clinical manifestations, nursing
management, complications, and prevention. (Relate TB, pleurisy, pleural effusion, and empyema to
pulmonary infection.)
- Use the nursing process as a framework for the care of patients with pneumonia and TB.
- Describe nursing measures to prevent aspiration.
- Describe the pathophysiology, clinical manifestations, medical and nursing management of chronic
obstructive pulmonary disease (COPD) and asthma.
- Discuss the major risk factors for developing COPD and nursing interventions to minimize or prevent
these risk factors.
- Use the nursing process as a framework for care of patients with COPD and asthma.
- Develop an education plan for patients with COPD and asthma.
- Describe COPD and asthma self-management strategies.
What is work of breathing?
- Describe the physical assessment of the respiratory system?
- Basic Respiratory Overview
- Work Of Breathing
o Retractions
o Accessory muscle use
o Flail chest
Significance of respiratory pathologies
- A, B, Cs
o Cannot have B without A
o Cannot have oxygenated C without B
- Need to assess and intervene quickly
o Oxygen therapy is mainstay of initial treatment
Source at every patient bedside in the acute setting in the U.S.
Follow hospital protocol on what oxygen delivery device is at every bedside – usually bag
mask for emergencies, some will also place nasal cannula at bedside
Make sure you know where supplies are and the oxygen Christmas trees
o Advanced airways when no other options
o Medications to reduce infection or inflammation
