CC: “I have been experiencing pain on my right testicle.”
HPI: A 30year old man presenting a complaint of pain on the right testicle. He states that the
pain began five days ago and the pain worsen when he sits down and when he lifts the
scrotum. He also states he is vomiting and has increased urinary frequency. He denies having
been involved in any vigorous activity. He states 4weeks ago he went on a business trip to
London and had unprotected sex with a female colleague.
Medications:
One a day men multivitamins
Denies any fold or herbal medicine
PMH
Allergies:
No known drug allergies
Allergic to cats
Medication Intolerances: None
Chronic Illnesses/Major traumas Patient has no significant chronic illness. Denies trauma.

Immunization He claims he is up to date with all the immunization and plan to get his flu shots
next week.
Environmental hazards. Patient states he works in steel manufacturing company. He lives with
his wife and their two children in a clutter free and clean home environment. He states at
home he gardens at times and also plays football with his son when free.
Safety measures: Patient claims he always wears his safety belt and also wears protective gears
when he visits the manufacturing department when at work.
Exercise and leisure patient says he jogs on weekends and also goes for hikes once a month.
Sleep Patient states “roughly I sleep for 6hrs, I go to bed at 10pm and wake up at 5am”.
Diet Patient claim he takes fairly healthy meals. He also drinks caffeinated beverages and
drinks about a litre of water a day.
Family History Mr P. R’s father is 70years and he is says he was recently diagnosed with
diabetes and his mother is overall healthy. His grandparents are all deceased and both his
grandmothers died from cancer and one of his grandfather from his father’s side died from
uncontrolled hypertension.
Social History The patient works as a manager in a steel manufacturing company. He is married
and has two children. He admits having another sexual partner who is his colleague at work.
