Florence Blackman, a 50-year-old female patient, presented to the clinic with a complaint
of intermittent squeezing chest pain. She noted pain in her mid-chest, which radiated to her left
arm, and experienced shortness of breath during her morning skiing run. She was exercising in
freezing weather. However, the pain declined when she stopped to rest, and the pain subsided
when she skied less aggressively in the subsequent days. After the skiing holiday, she resumed
her regular exercise schedule and did not experience any pain under typical warm weather.
However, the sharp chest pain recurred while she was exercising during cold weather, which
often caused her to rest. When resting, she experienced no pain. The fluctuation of the condition,
especially the chest pain, forced her to visit the clinic.
History
The patient states that she has no drug allergies. She had two pregnancies previously,
none of which survived beyond 24 weeks. She once has mild hypertension and suffered
hypercholesterolemia, and is on medication. She considers herself overweight. The patient
undergoes regular cancer screening and health checks. She has had no abnormalities and checks
her blood pressure every four months. Overall, the patient is physically fit because of threetimes-a-week aerobic exercise. She mainly eats out and prefers fast foods and strives to have a
healthy diet when at home. She manages stress through skiing and aerobics. The patient’s
immunizations are up to date. Her father died aged 65 due to stroke, while the mother died at 54
years of unspecified heart disease. Her 54- year-old sister recently had open-heart surgery. Mrs.
