● Depakote – given to bipolar patients
◆ Antiseizure and anticonvulsive medication
◆ Enteric coated – can’t crush, cut, or chew medication
● Useful agent in
◆ rapid cycling,
◆ substance abuse,
◆ aggression,
◆ intermittent explosive disorder
● Divalproex Sodium - Depakote® - (enteric coated)
● Valproic Acid Syrup – Depakene® syrup
● Valproic Acid – Depakene® (250 - 1,500 mg/d)
● Therapeutic Range: (50 – 80 µg/mL) for Mania (80 – 125)

● Client complain swallowing tablet:
◆ Call provider and change to syrup
● Can be syrup or dissolving tablet
● Side effects:
◆ GI distress N/V/D - give with food
◆ Drowsiness -give at bedtime
◆ Tremors – Propranolol
◆ Alopecia – zinc, multivitamin with B complex and
selenium supplements
◆ Weight gain - diet and regular exercise
◆ Increase plasma ammonia – neurotoxic = blunted, drunk
feeling, delirium
◆ Black box warnings: Hepatotoxicity, pancreatitis,
teratogenicity (Category X). Contraindicated for child
bearing women

● Therapeutic Range: (4 – 12 mL)

● Should know the levels should be done weekly for the first 8
weeks of treatment
● Anticonvulsant, bipolar disorder, trigeminal neuralgia, severely
paranoid
● Side effects:
◆ sedation,
◆ dizziness,
◆ anticholinergic,
◆ rash,
◆ less weight gain,
◆ hepatic dysfunction,
◆ leukopenia (rare),

■ Lamotrigine (Lamictal)
● A patient is at risk for steven-johnson syndrome
● Will be a rash, typically on the chest, back or lip area (if lips, will
appear as blisters)

◆ Tell patient to stop taking and to go to the ER
● Slow titration process:
◆ (25 mg for 2 weeks, then 50 mg for 2 weeks, then 100 mg
for 2 weeks)
◆ Generally effective at 100 to 200 mg up to 400 mg
◆ Takes MONTHS to get to therapeutic levels, so take with
other anticonvulsant
● Side effects:
◆ Rash/blisters (Stevens Johnsons) = on lips, or chest
