Important questions to ask the patient to formulate the History of Present Illness include asking about the nature of feelings that she experiences such as guilty feelings, feelings of worthless and also how the patient can rate her reduced interest in work and other activities that she was initially interested in. It is also important to ask the patient to rate severity of the symptoms she experiences (Carvalho, Firth & Vieta, 2020). This can be done by offering a rating scale 1-10 to help in determining how severe the symptoms are. In this case, it is important to carry out and review a drug toxicology testing. This examination will help in determining if the patient shows any drug presence in the blood which could be the reason for her recent change of behavior. It is also important to carry out a safety and self-care assessment on the patient (Yatham et al., 2018). This examination will entail evaluating the patient’s suicide and violent risks and also the patient’s ability to undertake her normal routine activities. The examination will help in determining is there is a need to hospitalize the patient or not as well as determine the right care plan. Having a discussion with the patient’s family is also important in helping formulate the right diagnosis. The discussion will provide information on if the family has a history of any mental illnesses as well as provide more information on the patient’s conduct while at home. It is important to discuss with the family the deterioration to self-care as portrayed by the patient (Carvalho, Firth & Vieta, 2020).
