The important questions to ask the patient to formulate the HPI include the start of his shortness of breath (SOB), what makes it better or worse, medications that he is taking, whether he has experienced weight gain, when his swelling started, and his diet among others. 2. What components of the physical exam are important to review in this case? What are pertinent positive and negative physical exam findings to help you formulate your diagnosis? The components of the physical exam that are important to review in this case include the temperature, skin, hair, nails, eyes, mouth/pharynx, jugular venous pressure, chest and lungs, blood pressure, and respiration among others. The pertinent positive findings that helped in formulating the diagnosis include hypertension, swelling of less and abdomen, bloating of the abdomen, tachycardia, and fatigue.
