8. 1 Acute Stroke unit care recommendations
● Admitted patients to hospital with an acute stroke/transient ischemic attack should
be treated in a inpatient stroke unit ASAP
○ Ideally within 24hrs from arrival
○ Stroke unit should be a specialized and geographically defined hospital unit
(specialised area with trained professionals)
○ For those without stroke units, they must strive to focus care according to the
comprehensive stroke care delivery
■ Includes:
● Clustering patients
● Interdisciplinary team
● Access to early rehabilitation
● Stroke care protocols
● Case rounds
● Patient education
● Core interdisciplinary team on the stroke units should consist of healthcare
professionals with stroke experience. This includes:
○ Physicians
○ Nurses
○ Occupational therapists
○ Physiotherapists
○ Speech-language pathologist
○ Social workers
○ Clinical nutritionists/dietitians
■ Pharmacists should be included to promote patient safety, medication
reconciliation, patient/caregiver/family and team education about
medication(side effects, adverse effects, drug interactions) and should
be included for discussion about adherence and discharge planning
■ Additional members can include discharge planners, case managers,
neuropsychologist, palliative care specialist, recreational and
vocational therapists, spiritual care providers, peer supporters, and
stroke recovery group liaisons
