Based on the expert feedback from the case study, Ms. Mabel Johnson
is diagnosed with osteoarthritis (OA) and her right knee is progressively
worse compared to her left. Her disease history of peptic cancer could not
allow her to use non-steroidal anti-inflammatory agents and instead receives
a medication, which assisted her to have pain relief such as Acetaminophen,
which did not bring much change. She was injected with corticosteroids for

pain relief. She later underwent an evaluation of orthopedic for analysis of
total knee replacement, which was successful and now takes acetaminophen
and physical therapy to manage occasional pain. According to Migliore et
al., (2019), osteoarthritis (OA) remains a common rheumatic disease that
remains prevalent among the adult population of eleven 11 percent for hip
and 24% for knee OA. Among the elderly, OA is a common cause of disability
comprising pain, limitations of activities and social contribution. With
increasing life expectancy, the number of people suffering from the
condition might increase.
After reviewing the expert feedback, I have discovered that the
correct diagnosis of the disease should encompass a classification of the
problem category, which was musculoskeletal and the selected differential
diagnoses identified as arthritis, rheumatoid. However, the missing aspects
were the diseases specified by the case author. Therefore, according to the
feedback, there was a need for differential diagnosis of the pseudogout
condition (Sarii et al., 2019) which can present with redness and warmth,
pain and swelling.
The referenced article has greatly assisted to learn about evidencebased recommendations and guidelines for managing knee OA. The article
has listed these options and addressed how patients and physicians can get
satisfaction from managing the condition. The article calls for a change in
therapeutic strategies. From the article, I have learned about a theory that
can support clinical decision making such as in our case study. The article
discusses the treat-to-target principle, which is grounded upon defining a
specific target and has been developed together with a comprehensive
therapeutic plan targeted at achieving low disease activity via a tight
control of the disease.
The theory will answer the question of whether patients and
physicians are satisfied with the current KOA management. I will use the
information from the article in my future intervention to count the number
of swollen and painful joints, genetic inflammation indexes and general

patient status in the everyday practice of measuring disease activity. The
evidence-based study is of great importance as it will assist me to find a
therapeutic target that considers disease activity, severity and attaining the
expectation of the patient.
