DRUG CARD Medication (Generic and Trade): clarithromycin/Biaxin, Biaxin XL Classification(s): ● Chem class: Macrolide ● Func class: Anti-infective Pharmacokinetics: Regular release: peak 2-4 ... hours; duration 12 hours; extended release: peak 5-7 hours; half-life 5-7 hours; metabolized by liver; excreted in bile and feces; possible inhibition of P-glycoprotein; protein binding 70% Action: Binds to 50S ribosomal subunits of susceptible bacteria and suppresses protein synthesis. Uses: Mild to moderate infections of the upper and lower respiratory tract, uncomplicated skin and skin structure infections caused by streptococcus pneumoniae, mycoplasma pneumoniae, legionella pneumophila, Moraxella catarrhalis, Neisseria gonorrhoeae, Corynebacterium diphtheriae, listeria monocytogenes, haemophilus influenzae, streptococcus pyogenes, staphylococcus aureus, mycobacterium avium complex (MAC); complex infection in AIDS patients; mycobacterium avium intracellulare, helicobacter pylori in combination with omeprazole, H. parainfluenzae Unlabeled Uses: Endocarditis prophylaxis, dyspepsia, gastric ulcer, legionnaire’s disease, pertussis, SARS Contraindications: Hypersensitivity to this product of macrolide antibiotics, torsades de pointes, QT prolongations ● Precautions: Pregnancy, breastfeeding, geriatric patients, renal/hepatic disease, heart disease Nursing Considerations: ● Assess: Infection: wound characteristics, urine, stool, sputum, WBC, temperature; C&S before product therapy; product may be given as soon as culture is taken; C&S may be repeated after treatment; Bleeding: check INR if anticoagulants are taken; Hypersensitivity: allergies before treatment, reaction to each medications; Heart Failure: increased risk of heart related disease and death in some patients, use caution in those with increased CV risk and weight benefits and risk; Clostridium difficile-associated diarrhea: monitor for diarrhea, cramping, blood in stools, fever, report immediately to provider; Serious skin reaction: product should be discontinued immediately. ● Evaluate: Therapeutic response; C&S negative for infections, prevention of endocarditis Patient Education: [Show More]
