NSG 6435 Peds Week 3 discussion Discuss the history that you would take on this child in preparation for the well-child visit. Include questions regarding her growth and development that are approp... riate for her age. A 2-month-old female, who is present for a well check and immunizations accompanied by her mother. Initial visit consists of pregnancy and birth history, any previous illness or issues before the visit, medical/family history, previous visit notes, developmental, dietary, social, and vaccination history. In addition, to the collection of historical information a complete head to toe assessment. The infant’s dietary assessment should include her feeding habits, if she is on breastmilk, formula, or both, frequency of feedings, and if she has any difficulties. Assessment of growth and development will include comparison of current weight, lengths, and head circumferences, gross motor, fine motor, language, and social development for a 2, 6, and 9-month-old visits. Appropriate question at 2, 4, and 9 monthsinclude (CDC, 2018): 2 months 4 months 9 months Is she able to lift her head and shoulders while on stomach? Does she smile spontaneously? Does she make different sounds? Can she follow an object to midline, past midline? Does she copy facial expressions and babble? Does she copy sounds and gestures? Does she respond well to sounds? Does she reach for objects with one hand? Is she able to watch the path of something that falls? Does she recognize and pay attention to faces? Does she hold her head steady unsupported? Does she move things smoothly from one hand to the other? Does she smile when spoken to or in response to your smile? Is she able to roll from tummy to back? Can she sit without support? Does she coo or make gurgling sounds? Is she able to push up on elbows when lying on stomach? Can she crawl, pull to stand, and/or cruise? Does she calm herself, by bring her hand to mouth? Does she recognize familiar faces and objects from a distant? Does she understand no? Describe the developmental tool to be used for Asia, its reliability and validity and how Asia scored developmentally on this tool. Is she developmentally appropriate for her age? Developmental screening and surveillance were utilized instead of a developmental toolfor Asia’s 2 and 6-month visits, recommendations for such tools are for 9, 18, 24, and 30 months. According to Aquifer (2018), screening and surveillance are not as sensitive or specific as a validated developmental screening test. On Asia’s 9-month visit, the PEDS developmental screening tool was used. The PEDS screening tool involves parents in the processand is used to aide in the identification of developmental delays (Brothers, Glascoe, & Robertshaw, 2008). At 2 months, she was able to lift her chest off the table with her head held up 90 degrees, follow pass the midline, and smiled spontaneously. At 6 months, she could pull to a seated position, sit without support, grab block and transfer it to the other hand, and babbles. At 9 months, she was able to sit unsupported, pull herself to stand, and crawl. As demonstrated with the screening, surveillance, and PEDS screening tool Asia is meeting her developmental milestones appropriate for her ages at 2, 6, and 9 months. What immunizations will Asia be given at this visit; what is the patient education and follow-up? Asia received her initial Hepatitis B vaccine after birth upon discharge from the hospital. Prior to administering vaccination information handouts are provided, explained, and understanding is verbalized, then consent is signed. Education is provided to Asia mother regarding the possibility of redness, tenderness to site, irritability, and the development of a fever. During Asia’s 2-month visit, she received her second dose of Hepatitis B, and her first doses of RotaV, DTaP, Hib, PCV13, and IPV vaccinations and instructed to follow-up in four months for her 6-month well check and immunizations or as needed (Centers for Disease Control and Prevention, 2018). At her 6-month visit, she will receive her third doses of DTaP, Hib, HepB, RotaV, and PCV13and follow-up is made for 3 months for a 9-month well check or as needed. References Aquifer. (2018).
