I have also had a lot less energy than before a... nd I don’t know why.” HPI: M.B. is a 23-year-old Korean American female who presents to the clinic with a three-month history of abnormal, heavy menstrual bleeding, extreme fatigue, weakness, headaches, palpitations, and intermittent shortness of breath. She denies backaches, abdominal pain. PMHx: Denies PSHx: Adenoidectomy. Denies any other hospitalizations or surgeries. Allergies: KNDA, KNFA Vaccinations: Up to date. Family Hx: Both sets of maternal and paternal grandparents still alive. No known medical history. Mother: Alive; no medical problems. Father: Alive; pre-diabetic. No siblings. Social Hx: Patient states she is single with no children. M.B. is currently in graduate school and works part time at the local Emergency Room as a Registered Nurse. She routinely donates blood and plasma at work. Patient is sexually active and states she uses contraceptives. She denies smoking and illicit drug use but admits to occasional social drinking. She participates in Olympic lifting 4-5 times per week. Drinks one Starbucks coffee a day. Eats a dominantly Korean diet high in fish, meat, and vegetables. Sleeps 6-7 hours a night.
