Case Study: Amy Kellar Associated Course Competencies: • Utilize the appropriate technology and/or references/resources, demonstrating accurate use, in order to access reliable data and information that support evidence-based practice in the care of diverse clients across the lifespan. • Differentiate the various components of basic physiological needs as it relates to fluid/electrolyte and acid/base balance, including but not limited to associated concepts of elimination, immunity, thermoregulation, and infection control, in order to facilitate the promotion, maintenance and restoration of health and wellness. • Evaluate data and information gathered during patient care, simulated scenarios, and/or case studies related to gas exchange and pain management in order to determine knowledge and wisdom gained through critical thought processes to optimize patient outcomes and quality improvement. • Demonstrate a basic understanding of communication practices necessary for patient-centered care and interdisciplinary collaboration in terms of knowledge, skills, and attitudes. Client Profile • Gender: female • Age: 27 yo • Weight: 130 lbs • NKDA • Setting: hospital • Ethnicity: white American • Cultural Considerations: none identified • Past Medical History: several Urinary Tract Infections (UTI) over the past three years ago • Communication: English • Disability: none • Social: married; stay at home mom; one daughter (9 mos); husband works 2 jobs Chief Complaint / History of Present Illness Ms. Kellar c/o sudden onset abdominal pain approximately 12 hours ago. She describes the pain as a constant ache in the lower right quadrant (RLQ) of her abdomen that radiates across the flank to her lower back. She rates her pain as a 9/10. She also c/o nausea and has vomited three times prior to arrival. She is febrile and c/o chills. During your initial history, Ms. Kellar tells you that she has had urinary frequency with “darkish funny smelling urine for a few days.” She also recalls a lower back ache for several days and feeling “hot and cold.”
