NURSING MISC EXAM 4 STUDY GUIDE
Page 1264 – 1270 Hyperthyroidism
- Excessive thyroid hormone secretion from the thyroid gland
- Symptoms of hyperthyroidism = thyrotoxicosis
- Thyroid hormones increase metabolism in all body organs – this could cause hypermetabolism, & sympathetic nervous system activity
- Thyroid Gland – releases T3 & T4: needs iodide to make T3 & T4
T3 – T4:
controls metabolic rate: protein, carbohydrate, and fat
respiratory
cardiac rate
Glucose tolerance
Libido
Risk Factors: causes
- Grave’s Disease: autoimmune disorder resulting from Hashimoto’s thyroiditis (HT) – antibodies attach to TSH receptors on the thyroid gland, which the number of glandular cells = the gland = forming GOITOR (Enlargement of thyroid gland, can happen with hyper & hypothyroidism) &over production of thyroid hormones = thyrotoxicosis
o Women from 20 – 40 yrs.
o S/S – general symptoms of hyperthyroidism and …
Exophthalmos (abnormal protrusion of eye, shocked look)
Pretibial myxedema – dry, waxy swelling of the front surfaces of the lower legs – looks like tumors or keloids
- Toxic multinodular goiter – large thyroid tissues or benign tumors
o Usually after 50 yrs. Affects women 4x more than men
o Pt usually have had goiter for years
S/S- milder than graves, no exophthalmos or pretibial myxedema
- Excessive use of thyroid replacement hormones – exogenous hyperthyroidism
Assessment: T3 T4 = cardiac, metabolism, glucose & respiratory
- History – gather age, gender, usually weight – unplanned weight loss, appetite
- HEAT intolerance – increased sweating even when temps. Are comfortable
- Cardio – ask for breathing changes – dyspnea at rest or on exertion is common
- Vision – blurred vision, eyelid retraction/ lag – ask pt to look down and up
- Observe size & symmetry of thyroid gland
- Psychosocial – mood swings, irritable, attention span, manic behavior
o Hyperactivity = fatigue – can’t sleep well
o Cries or laughs with no reason
