They will perceive their office visit much better, more patient satisfaction, more likely to adhere to treatment options if you discuss sexual health. Women are uncomfortable bringing up sexual dysfunctions and don’t know it is appropriate to ask for help. They are desperate for you to ask. First off, you can use a screener in waiting room. i.e. for low sexual desire (most common sexual problem). Decreased sexual desire screener containing 5 questions which can help with diagnosis and rule out all other factors such as medical condition, relationship factor, or stress. Simply ask open ended questions, what sexual concerns do you have? Do you have concerns with desire, arousal, orgasm, or pain? This will get you all key categories. Then you can make some treatment options for them. Just ask, normalizing, validating their right to sexual health already makes them satisfied with office visit.
Genitourinary syndrome of menopause (GSM) incorporates VVA. It describes estrogen deficiency, changes of the vulva and vagina. Whole syndrome that involves symptoms of the vagina and urinary tract and sexual symptoms. Symptoms need to be bothersome and not accounted for by anything else but estrogen deficiency for a woman to have a diagnosis of GSM. The most common reason for painful sex after menopause is GSM. Up to 80% of women will have symptoms of GSM and in those women, over 50% will have bothersome pain with intercourse. It is important to rule out other diagnosis such as lesions in the vagina that would need to be seen on exam and treated, other anatomic or structural change such as
