He was on a camping trip with his family when he accidentally fe... ll on the campfire causing severe second- and third- degree burns over 60% of his body. The clinical care team tells his parents that it will be critical to maintain their son’s airway and keep his fluid levels high. 1. The father is confused and asks you why his son’s fluid level is important after a burn. How would you explain this to him? Tissue injuries, like burns, increase capillary permeability. This allows plasma proteins to leak (where proteins go, water/fluid follows). This decreases capillary colloidal pressure, which normally helps the body keep fluid in the vascular space and increases tissue colloidal pressure, which causes an increased “pull” in fluids from the capillaries/vascular system. When fluid is in the interstitial space it is no longer functioning to maintain body systems. Sometimes this can be complicated further if the lymph system cannot keep up and lead to third spacing of fluids. In third spacing, fluid can collect in the pericardial (heart), pleural (lungs), or peritoneal (abdominal) spaces.
