Mrs. Pham is on very good health. She lives at home with her parents and 15- year old sister. Although she feel happy and comfortable living with her parents she would like to move away and having some independence. She states being in a good health and denies any serious illnesses, allergies and or any medication treatment. Mrs. Pham in her free time uses to go to movies, baseball games and sometimes have a party with close friends. She drinks alcohol occasionally and do not smoke. Mrs. Pham do no use to have a stable relationship, but recently she met a nice guy who would like to know better. Patient states feeling safe at home and denies any depression or anxiety. Through the interview it was found that the patient missing the three HPV vaccinations and tetanus-diphtheria-acellular pertussis vaccine. In this first visit the TDAP vaccine was administered and she will come back for the HPV vaccine in the next consult. Safety education was provided. Three months later, Ms. Pham return to the clinic with urinary symptoms such as dysuria, urinary frequency, suprapubic pain, white vaginal discharge, and hematuria started 2 days ago. Pt. Denies fever, chill or vomiting. Pt has a history of having a recently unprotected sex with her boyfriend. Labs was performed and it was a positive result for acute cystitis. Other labs were performed to rule out STI. Some labs are pending. Treatment, education and follow up was provided in this consult. The day after this visit the pending labs results were obtained with a positive result for chlamydia infection. It was offered treatment, education and follow for the infection.
