Mr. Jones Case Study Jennifer Washington Chamberlain College of Nursing NR 601 Primary Care of the Maturing and Aged Family The purpose of this case study is to evaluate pertinent data provi... ded to develop an appropriate treatment plan for Mr. Jones. Mr. Jones is a 60-year-old African American male that presents to the office for a six month follow up visit for hyperlipidemia and weight loss. Using evidence-based guidelines, this paper will analyze the details of case and develop a plan. Assessment Primary Diagnosis: Diabetes Mellitus Type 2 (E11.9) Pathophysiology – "Type 2 diabetes is characterized by insufficient circulating insulin, insulin resistance, and an inadequate compensatory insulin secretion response" (Dunphy, Winland, Porter, Thomas, 2015). Hyperglycemia is toxic to the beta cells in the pancreas. "These beta cells are responsible for the production and storage of insulin" (Dunphy et al., 2015). Pertinent positive findings – Pertinent positive findings for Mr. Jones include his increased thirst and increased fatigue, increased appetite, inability to lose weight, and increased urination during the day and at night. Besides, his lab work shows a hemoglobin A1c of 6.9% and a fasting blood glucose of 135, which are outside of the normal range (American Diabetes Association, 2019). “The national diabetes guidelines state that a hemoglobin A1c greater than 6.5 is indicative of diabetes” (ADA, 2019). Mr. Jones’s age, weight, and ethnicity are risk factors for diabetes. Pertinent negative findings – Mr. Jones's parents are deceased, and the case study does not state he has a family history of diabetes. He does exercise. He completes physical activity for at least 30 minutes three to four times a week. He also denies blurred vision. [Show More]
