This note will include all pertinent information regarding the patient signs and symptoms, diagnostic test results as well as the treatment plan. Assessment Primary Diagnosis: Diabetes Mellitus type 2 (ICD-10 E11). pathophysiology: In type 2 diabetes mellitus, insulin secretion is reduced due to impaired ß-cell function (ADA, 2018). Some common signs and symptoms of diabetes type 2 include and are not limited to being thirsty and hungry all the time, frequent urination, increased fatigue, and slow wound healing (ADA, 2014). pertinent positive findings: According to ADA (2018), there are some criteria in order to diagnose a patient with diabetes. These include the A1C, fasting glucose, and the 2-hr plasma glucose value during a 75-gram glucose tolerance test. Mrs. Wu’s fasting plasma glucose was 130 as well as has a Hgb A1C of 6.8%. Additional positive findings include 1+ glucose in urine, increased fatigue, increased hunger and thirst as well as increased frequency of urination during the day and night (ADA, 2018). pertinent negative findings: Some of the negative findings for Mrs. Wu and her diagnosis for diabetes mellitus include: weight gain, normal renal function, absents of protein and/or ketones in urine (Garber et al., 2018).
