Mr. Jones’ Case Study Chamberlain University Introduction The aim of this paper is to examine subjective and objective data of a case study to properly diagnose and develop a management plan utili... zing evidence-based practice guidelines. This case study involves Mr. Jones, a 60-year-old African American male who presents to the office for a six month follow up visit for hyperlipidemia and weight loss. However, during while obtaining the history of present illness (HPI), he complains of increased fatigue, weight gain, polyphagia, and polyuria. Assessment Primary Diagnosis: Type 2 diabetes mellitus (E11.0) Pathophysiology: The pathophysiological basis of type 2 diabetes mellitus is defective insulin secretion, meaning the body either does not produce enough insulin or the cells ignore the insulin (Skyler et al., 2017). This disorder usually begins with insulin resistance, where the cells do not use insulin properly, and as the need for insulin increases, the pancreas attempts to overcompensate by producing more insulin, but eventually, the pancreas loses its ability to produce the insulin (Skyler et al., 2017). Signs and symptoms include frequent urination, increased thirst, always feeling hungry, and tingling and/or numbness in hands or feet (Skyler et al., 2017). [Show More]
