We were discussing our current assignments, and she shared that she had attended a lecture a few years back, one of the speakers lectured on the Bureaucratic Caring Theory. As I reached this theory, I found that it encompassed caring, which I am extremely. Caring is the central core surrounded by the complex systems of bureaucracies that include political, legal, economic, technologies, and social-cultural dimensions (Smith & Liehr, 2018). Brief summary of the nursing theory I chose including key concepts: Dr. Marilyn Ray, a nursing theorist, researched caring and found that caring involves treating others as human and to consider one’s social, educational, ethical, and spiritual qualities during interactions. Dr. Ray developed the theory of bureaucratic caring in 1981. The original framework displayed caring at center surrounded by layers of the complex systems in a circle outside of the caring structure. The complex systems of educational, ethno-religious humanistic, economic, technology, legal, and political. Later, Dr. Ray did further research and designed a new framework that incorporated the ethical and spiritual components along with caring in the center of the circle to show the importance of the three within the healthcare organizations. How does the selected nursing theory address or identify the concepts of the nursing metaparadigm? The bureaucratic caring theories addresses the 4 concepts of nursing metaparadigm person, health, nursing, and environment. Person: Patient's are care for holistically taking account of their spiritual needs. Caring is the central theme that ties ethical, spiritual, and social needs. Individuals engage co-creatively within organizational and transcultural relationships allowing them to feel valued (Johnson, 2015) Health: Health is harmony, unity, relationships, and the connection of one's experiences with the social and health organizations. Nurses are instrumental in disease prevention and health promotion. When we account of the patient's spiritual cultural, financial needed we can reduce stressors for patients allowing to force on their health Nursing: Provide holistic, spiritual care, and ethical care while maintaining self-care. Nurses operate centrally between organizations, communities while adhering to bureaucratic constraints. Environment: Nursing embodies the complex concepts of the bureaucratic caring theory while cultivating a positive, caring experiences for the ill. Nurses adhere to ethical and fiduciary standards while maintaining positive professional relationships with other members of the healthcare team. How is this nursing theory useful to the nursing profession? The theory of bureaucratic caring is useful to the nursing profession as it provides a framework to foster the nurse to patient and the administrator to nurse relationships. Dr. Ray’s research has shown how nurses, patients, and administrators can use caring behaviors to transform organizations. Organizations that infuse trust and respect to their patients and staff can become more successful. In addition, a healthcare organization that respects that provide ethical timely, and appropriate care can be more financial sound because procedures will be done in the correct setting not done just because the patient is in the hospital. As a nurse case manager I have been faced with many ethical dilemma with determining if a patient is safe to go home and I’ve had to addresses not feeling that patients are safe for discharge or discharging to a safe situation. The bureaucratic caring theory addresses the need for healthcare providers to practice self- care as well as care for our patients. In order to achieve caring to patients we must take time and care for ourselves by finding time to relaxing with family and friends, enjoying hobbies such as sports, crafts, shopping, cooking, listening to music, and taking time off to do nothing. We cannot care for others if we are depleted. The empirical evidence seen in this grounded theoretical approach using qualitative research by Dr. Ray show caring in all areas of a hospital from nursing practice to material management to administration supports that nurses should focus on the good of others and never treating patients as a means to an end. How would I apply this nursing theory to my chosen MSN specialty (educator, leadership/executive, nursing informatics, healthcare policy, nurse practitioner, population health, etc.) As a future healthcare policy professional I would apply the bureaucratic caring theory by working with hospital, local, state and the federal government to ensure that nurses are protected and can provide safe effective care to patients. The current COVID-19 pandemic have shown how the system is completing failing our healthcare professional as well as our patients. The lack of personal protective equipment is a failure on so many levels and this is not the first time. In 2008-2009 with H1N1 there was a shortage of N95 mask, did that not teach us anything. This is a complete breakdown of the complex system of educational, ethno-religious humanistic, economic, technology, legal, and political. I will push for increased education in emergency preparedness plans, and drills, stock piles of personal protective equipment on the federal levels but on state, local and hospitals. As well an increase of life saving and sustain equipment. I would also lobby for hot zone medical staff be fitted for personal respirators that would decreases the use of N95 mask. Nurse and other healthcare professionals must feel cared for while providing care and not feel beholden to bureaucratic failures. Self-Reflection: What did I learn from completing this assignment? In completing this assignment my eyes were open to the parallel of caring and bureaucracy. I have gained a deeper understanding of the theory of bureaucratic caring through researching this topic. As nurses we should take the time to provide a caring moment in our personal and professional lives and to treat others as human no matter the political or financial implications.
