NR 511 Week 6 Case Study Discussion: Part One.History of Present Illness The patient is 56 yr. old female who presents to the clinic today with CC of fatigue. Fatigue onset was 2-3 months ago, and it... is generalized and constant. The fatigue is progressively worsening, and the patient feels tired all of the time. She sleeps 8 hours per night but does not feel rested. Exertion aggravates the fatigue, and no relieving factors have been identified. The patient states that the fatigue has been severe enough for her to miss work because she “could not get out of bed.” Along with fatigue, the patient reports constipation, generalized weakness, muscle cramping in calves, cold intolerance, and worsening depression. Physical Assessment Pertinent patient physical assessment findings are dry skin, thick coarse hair, and decreased deep tendon reflexes in bilateral lower extremities. Also, her thyroid gland is small and firm.
