Abstract The purpose of this paper is to describe an evidence-based proposal project. Falls and falls with injuries remains the most costly of adverse events. While no one can completely prevent a ... fall from happening, there is the potential to accurately assess the patient’s risk level for falls. Using Hester Davis Scale for Falls Risk Assessment (HDS) can potentially decrease the rate of falls, but especially reduce the rate for falls with injury. Working closely with the Hester Davis team to successfully implement the HDS, Kaiser Permanente (KP) Southern California Region intends to see a decrease in all falls and especially falls with injury.
