Hospital Discharge Plan Title Student's Note Write what you would say to instruct Tina about managing her diabetes. Ms Jones, before we can discharge you, I would like to provide some important information regarding managing diabetes with you. It is important that you regular monitor your blood glucose at home and keep up with your medication schedule. Always feel free to reach out to our doctor's office if you have any concerns regarding your medications. Monitor your blood sugar at home at specific times, d a test before each meal and 1 or 2 hours after a meal. Before, during and after physical activity. Detecting signs and symptoms of hypo- and hyperglycemia are crucial as well. These symptoms can occur rapidly, thus detecting them early is favorable. Hypoglycemia is characterized by hunger, trembling, nausea, sweating and a fast heartbeat. When you notice these signs of low blood sugar, drinking fru juice or anything with a good amount of carbohydrates is recommended. Signs an symptoms of hyperglycemia include fatigue, increased thirst and urination. By managing your diabetes with the prescribed medications, we can prevent commo complications of diabetes. Diabetes could affect your vision; high blood sugar cou lead to blurry vision, cataracts and glaucomas. With these vision problems, an increase of headaches are inevitable. Diabetes could also affect your other organ high glucose over time can affect kidney filtration tissues and your arteries to stiffe which makes the chances of developing heart disease and kindey failure more lik Other complications include nerve damage, which are characterized by pain, numbness and tingling sensations in the hands or feet. When caring for your feet make sure to check for lesions and be careful with clipping your toenails, preferab get them done by a podiatrist to prevent any bleeding complications. Besides medication, I would like to stress the importance of regular exercise and diet. Lim foods that are high in sugar, salt and fat, make sure to track the amount of carbohydrates as well. Try smaller portions of food spread throughout the day. Create a meal plan with a variety of healthy foods, containing vegetables, fruits, grains, protein and non- or low fat dairy products.
