1) Have you gained or lost weight unintentionally despite normal appetite and exercise?
I did ask the question of whether the patient had any recent weight loss or gain and had
gathered the information regarding the patient’s recent 11-pound weight loss and decreased appetite. It
is important to ask about weight loss as this has been proven to be a significant predictor of cancer.
According to a recent meta-analysis (Nicholson, Hamilton, O’Sullivan, Aveyard, & Hobbs, 2018,) weight
loss was linked with atleast ten types of cancer and should guide the primary care provider’s decision for
urgent and further investigation.
2) Does your breast problem come and go?
Knowing whether the “lumps” come and go might direct the provider towards a diagnosis of
fibrocystic breasts. According to the American Cancer Society (2017), monthly hormone changes
often cause cysts to get bigger and become painful and sometimes more noticeable just before
the menstrual period.

There were no errors in my physical exam. I covered all the assessments the expert stated should
be covered but my examination involved many more assessments than needed for the patient’s
complaint.

The main key finding is the presence “lumps” as reported by the patient and the physical exam
consisted of a clinical breast exam that confirmed a palpable mass, palpable lesion, and palpable axillary
lymph node. According to Ravi and Rodrigues (2012, p. 154), “clinical examination is a simple method to
detect breast lumps and their nature as it is inexpensive and non-invasive and if found to be accurate,
might be of great value as a diagnostic tool”.
