NR 511 REFLECTION
Over the past eight weeks I have had the opportunity to participate in the care of many patients
with a variety of medical problems. I was able to find an internal medicine provider who has many
patients that enable me to understand how complex, challenging, fast-paced, and spontaneous primary
care can be. Prior to this experience the last 5 years of my experience as a RN was in the acute care
setting; I did have prior experience in outpatient adult medicine but I didn’t have access to the complexity
of being a provider that I do now as an FNP student. This experience has been enlightening, challenging,
and rewarding. Disease management and prevention are key to keep our patients healthy, safe, and alive.
As providers we are helping to prevent the onset of many disease but we are also providing solutions and
interventions that keep patients from having worsening symptoms. I have found myself loving the
difference in environment as well; however I have also learned that this environment presents a different
array of challenges than in the acute care setting which has me wanting to step up to the challenge and
come up with creative ways to make patients preventative care better for the future.
The patients that I have encountered are multi-cultured with an array of backgrounds, concerns,
and health issues. Many of the patients we have encountered are poor and rely on government issued
insurance which I am learning has created a different slew of challenges that I had not even foreseen
working in acute care. My patients have been between ages 18 to 80 years old who suffer from diabetes,
heart disease, gastrointestinal, autoimmune, respiratory, and psychiatric disorders. Due to challenges with
insurance I haven’t performed many procedures at this clinic because many of the insurances require the
patients be referred to specialist in order to get most specialty needs done and for vaccinations the
insurance doesn’t cover the ins office vaccine so instead we have to write prescriptions for patients to get
their adult vaccinations done which I personally feel is a big problem especially for the population that we
are caring for who already doesn’t have access to many resources. I have found that it can be a challenge
trying to get patients immunizations, referrals to specialist, approval for procedures, or medications
because the insurance company requires so many challenges that patients often aren’t compliant.
