She has a productive cough with white sputum and has wheezing on inspiration and expiration. She states that co-workers have been sick with influenza A. Allergies: Strawberries-rash; erythromycin, severe GI symptoms Current Medications: Albuterol HFA 90mcg, Flonase 50mcg, Multivitamin, Zyrtec PMHx: Asthma, seasonal allergies, saw allergist 10 yrs ago and prescribed allergy shots for past 5 yrs and had great results, takes Zyrtec PRN, had Eczema as a child Surgeries: Cholecystectomy Hospitalization: childbirth 3 times Immunizations: current Family Hx: A 3 kids are healthy, 1 child has a sinus infection currently, PM- passed at age 80 from CHF, PF- passed at age 82 from lung CA that had metastasized to his brain, PGF-passed at 82 from a stroke, PGM- passed unknown causes, MGM- passed at 83 from HTN, multiple heart attacks and atherosclerosis, PGF-passed at 71 due to COPD Social HX: husband 3 kids, was in advertisement until she was laid off 18 months ago, currently working as a baker’s assistant (Artisan Bread Bakery). She goes to work at 4am every day. She is a social drinker, in her 20’s she smoked 1 pack/wk. for 3 yrs., no drug use, gets 6 to 7 hrs. of sleep/night, four to five days/week she exercises.
