Ms. Johnson is a 83 year old obese woman with a significant history of chronic renal disease, hypertension, and PUD due to ibuprofen use who presents with progressive knee pain, right worse than left,... with a small right effusion. The knee painis worse in the morning as is her finger pain, but gets better with mild movement. Her knees have limited ROM and weight bearing exercises make the knee pain worse, such that the patient is restricting daily living activities. The patient's hypertension is not currently controlled in spite of patient stated medication compliance 1) please give 3 diagnoses Primary Diagnosis and ICD-10 code: Also include any procedural codes. 3-5 Differential Diagnoses- Why? What made you select each one as a DDX? How did you rule out? This would be a good area to include references. 2)Additional laboratory and diagnostic tests: May be necessary to establish or evaluate a condition. Some tests, such as MRI, may require prior authorization from the patient's insurance carrier. 3)Consults: referrals to specialists, therapists (physical, occupational), counselors, or other professionals. If you are sending to hospital, what orders would you write for a direct admit? 4)Therapeutic modalities: pharmacological and nonpharmacological management. 5)Health Promotion: Address risk factors as appropriate. Consider age-appropriate preventive health screening. This is not related to the CC. This is a review of all recommended health screenings, ie cholesterol screening, mammography, PSA, etc. 6)Patient education: Explanations and advice given to patient and family members. 7)Disposition/follow-up instructions: when the patient is to return sooner, and when to go to another facility such as the emergency department, urgent care center, specialist or therapist.
