Case Study: Mr. M
In this case study regarding Mr. M, one may simply draw conclusions based on the
clinical manifestations and presented symptoms. One must however, take Mr. M’s history and
medical information into consideration to critically understand the possible disease processes
given the data collected during the nursing assessment and lab results. By carefully evaluating
the collected data, two medical diagnoses were considered. Mr. M may potentially be suffering
from sepsis related to urinary tract infection (UTI) and early onset of Alzheimer’s Disease (AD),
both of which will be addressed providing interventions for Mr. M and his family.
Clinical manifestations present in Mr. M
Based on the objective data presented, vital signs are within normal limits with an
important emphasis of HR above 90 and respiratory rate of 22. However, Mr. M is noted to have
cognitive deterioration in the past two months with increased forgetfulness, trouble recalling
things, increased agitation, aggressiveness, and fear. In addition, one should also greatly note that
Mr. M physically deteriorated quickly after the surgery that resulted with ADL dependency. As
lab results were collected, WBCs and lymphocytes are noted to be abnormally high, with
urinalysis positive for moderate amount of leukocytes and cloudy, with normal protein, ALT,
and AST levels (Mayo Clinic, 2019).
