Student: Mahmud, Syed Chief complaint: Headache History of present illness: Beth Brown is a generally healthy 16-year-old high school student who presents with a more frequent severe (8/10) throbbi... ng headache that started first 10-12 weeks back. It is usually unilateral (left) and preorbital in location. It stays almost a whole day when it begins. She reports associated symptoms of nausea and vomiting. She noticed visual auras before the headache starts. She reports that sleeping in a dark room is of some benefit for her current symptoms, whereas bright light and loud noise aggravate it. She also tried Advil, Excedrin, and Tylenol, but nothing relieves it entirely. It is often precipitated by sleep deprivation, chocolate, and junk food. She cannot do anything during the episode of headache. She denies any medication overdose. PMH is significant for headache, but that is mostly related to bad posture and controllable with OTC medicines; however, this new-onset headache is intolerable, nonrecoverable, and is not triggered by reading on the bed for a long time. Past Medical HistoryOther active problems: She has been had another form of headache that is associated with her reading/studying on a bed. She describes it as band-like across her forehead along with tensed neck and shoulder. This is not severe and is relieved by Tylenol/Advil and/or a hot shower.
