Does she have any other symptoms or concerns we should discuss
On what part of her body did the rash start and where did it spread:
Has she been having fevers: yes she has
When did her fever start: 3 days ago
Did she have any other problems two to four days before the rash started
What treatments has she had for her rash?
Does she have any allergies
Is she taking any over the counter or herbal medications
Is she coughing up any sputum
How high is her fever
Is her rash painful or itchy
What treatments has she had for her cough
How many stool diapers does your child have per day
How is her appetite? Any recent changes;

PHYSICAL EXAM
SKIN: warm and dry, maculopapular pink rash on the abdomen and extremities
HEENT: inspect eyes no ptosis erythema or swelling, conjunctiva pink no discharge
Examine pupils, inspect ears, look in ears
Inspect nose, look up nostrils, inspect mouth/pharynx
Chest Wall & Lungs
Visual inspection anterior posterior chest
Normal lung sounds
Normal heart sounds
ABDOMEN
Flat symmetric with no scars
No pain, tenderness, masses or pulsation abdomen palpation. No guarding,
rebound tenderness
EXTREMITIS
Visual inspect all extremities
Palpate extremities

Look for involuntary movements
2-year-old presents with mother, history per mother, the mother state the rash started this
morning on the abdomen and spread to arms and legs. Fever 3 days ago of 103F that has
resolved. Runny nose and cough that started 3 days. Immunizations are up to date.

Roseola is a viral infection that causes a rash that appears shortly after the temperature returns to
normal. The rash appears as a maculopapular eruption that spreads mainly to the trunk area and
will usually disappear in 24 hours. Associated symptoms include a mild cough and rhinorrhea.
Fever followed by a rash is specific to Roseola.
Management: Symptomatic treatment. Tylenol or ibuprofen for fever as needed.
Tylenol oral suspension OTC
SIG: take 5ml PO every 4 hours as needed
Max dose 25ml in 24 hours, do not exceed 75mg a day
Children's Advil oral suspension OTC
SIG: take 5ml PO every 6-8 hours as needed, max 20ml in 24 hours
