I believe that certain levels of the social ecological Model can be used even here in
Chicago especially in the southside since a lot of communities are undeveloped and they
have a poor educational system with not too many schools and resources. Because of this,
kids are kind of forced into joining gangs or joining violent groups just to feel that they
are part of something. There is also the problem of stereotyping, where people believe
that because an individual is part of a certain race he or she may end up as a violent
individual. I believe that here in Chicago we can implement all four of these stages.
The individual stage can be implemented looking at the education factors agent in
relationship with education - if young kids of a certain age are not going to school or they
go to school with poor resources or the parents don’t have the income necessary to send
their kids to a good school, then we should allow them to go to school somewhere else
outside their neighborhood and provide them with transportation so they can achieve an
education.
Then some kids might be in a troubled relationship with their parents and that can affect
them in the long term. I believe that DCFS should inspect families who are suspect of
having issues. Every school even have mandatory programs at where kids can meet up
with a psychologist and talk about their problems if there are any, and so adults will be
aware of their situation at home.
At the community level I believe that the city overall should give more money to
developed communities in order for them to restore schools and buildings and create jobs,
so people won’t go looking for them in other places.
On a societal level I believe that we, as people have a duty to treat everyone the same,
and not discriminate against anyone because there is a very big problem, especially in
Chicago in regards to job inequality where white people get the good office jobs and
African-Americans or Hispanics don’t really get accepted into this type environment and
get jobs mainly in construction, fast food, retail etc.
2) What is the intended approach of the Chronic Care Model (CCM)?
The chronic care model’s goal is to restructure medical care and create a partnership
between health system and communities. The objective of the study was to apply this
model in order to provide care for people who have diabetes since diabetes is a major
cause of heart disease and stroke. The CCM was critiqued based on their interventions
however based on the results of the model interventions are actually effective for
managing diabetes in the US.
The model had six components which were health system, self-management support,
decision support, delivery system design, clinical information system, community
resources and policies.
This combination of components was important in order to produce optimal patient and
provider outcomes.
