1) Review the Uruguay article: Improving Health System Efficiency. Focus on
the introduction and the conclusion of the article.
1. What did Uruguay want to accomplish?
They are committed to having universal health coverage for its
entire population.
2. What steps did they start with?
They created the National Integrated Health System and the
National Health insurance, that set the basis of effective and
efficient processes, economically and socially.
3. What changes did they implement to achieve their goal?
One change apart of the National Health fund, which a ceiling
(max) payment according to the estimated risk. The main change
was proposed as organizing levels of care to increase efficiency.
The changes are based on primary care/first-level care.
4. How did they evaluate their goals?
Using the management model as a framework.
5. What are the factors that affect the efficiency of a health system?
Institutionally and governance, payment mechanisms to providers,
and human resource policies
6. What measures did the reform of the Uruguayan health system put in
place?
Measures that allow a better use of resources. The National Inegrated
health system introduced new payment mechanisms. Also, a main change
has been directed to reorganizing medical work and working conditions
7. What was an important component of the Uruguay National Integrated
Health System?

My take away kessage is that though a process of internal reform,
the Uruguayan health system has improved their goal of universal
health coverage by creating and imposing new resources. A large
reform, was towards payment plans, and insurance reform. For
example goal 4 reflected creating high dedicated positions in
general medicine. This type of “performance payment” is supposed
to improve quality of care and efficiency.
