Last used cocaine 28/02/18: Admission 01/03/2018:  Patent selfaadmited: decompensated schizophrenia Medical background:  Not compliant with medicatons.  Admited for auditory command hallucinatons tellinn patent to harm self.  Visual hallucinatons shadow fnures with nrinninn faces.  Delusion personal connectons to various politcal leaders. 01/03/2018 –  anitated and annressive, respondinn to internal stmuli with thounht blockinn and latency. Commenced antpsychotc meds (rispoderone) 10/03/2018:  Patent ceased reportnn auditory or visuala hallucinatons.Lessdisornanised thinkinn.  No sinns of thounht blockinn or latency.  Able to minimise delusions and focus on actvites of daily livinn. Nursing management:  Assess for objectve sinns of psychosis.  Redirect patent from delusions.  Ensure medical compliance.  Help maintain behavioral control, provide therapy if possible. Assessment:  Good pronress, chronic mental illness, can decompensate if not on medicatons or abusinnsubstances. Insinht nood, judnment fair.
