Admited for auditory command hallucinatons tellinn patent to harm self.
Visual hallucinatons shadow fnures with nrinninn faces.
Delusion personal connectons to various politcal leaders.

Commenced antpsychotc meds (rispoderone)
10/03/2018:
Patent ceased reportnn auditory or visuala hallucinatons.Lessdisornanised thinkinn.
No sinns of thounht blockinn or latency.
Able to minimise delusions and focus on actvites of daily livinn.
Nursing management:
Assess for objectve sinns of psychosis.
Redirect patent from delusions.
Ensure medical compliance.
Help maintain behavioral control, provide therapy if possible.
Assessment:
Good pronress, chronic mental illness, can decompensate if not on medicatons or
abusinnsubstances. Insinht nood, judnment fair.
Discharge plan:
Discharne on Risperidone 4n ninhtly by mouth.
Risperidone 1 millinram available twice daily p.r.n for anitaton or psychosis.
