Read the case notes below and complete the writnn task which follows. notes: Hospital: St. Mary’s Public Hospital, 32 Fredrick Street, Proudhurst Ms Bethany Tailor Next of Kin: Henry Tailor (fath ... er, 65) and Barbara Tailor (mother, 58) Admission date: 01 March 2018 Discharge date: 18 March 2018 Diagnosis: Schizophrenia Past medical history:  Hypertension secondary to fbromuscular dysplasia  Primary hypothyroidism Levothyroxine 88 mcn daily Social background:  Unemployed, on disability allowance for schizophrenia.  History of polysubstance abuse, mainly cocaine and alcohol. Last used cocaine 28/02/18: Admission 01/03/2018:  Patent selfaadmited: decompensated schizophrenia Medical background:  Not compliant with medicatons.  Admited for auditory command hallucinatons tellinn patent to harm self.  Visual hallucinatons shadow fnures with nrinninn faces.  Delusion personal connectons to various politcal leaders. 01/03/2018 –  anitated and annressive, respondinn to internal stmuli with thounht blockinn and latency. Commenced antpsychotc meds (rispoderone) 10/03/2018:  Patent ceased reportnn auditory or visuala hallucinatons.Lessdisornanised thinkinn.  No sinns of thounht blockinn or latency.  Able to minimise delusions and focus on actvites of daily livinn. Nursing management:  Assess for objectve sinns of psychosis.  Redirect patent from delusions.  Ensure medical compliance.  Help maintain behavioral control, provide therapy if possible. Assessment:  Good pronress, chronic mental illness, can decompensate if not on medicatons or abusinnsubstances. Insinht nood, judnment fair.
