⦁ Medication modalities: nebulizers/inhalers, etc. to include teaching & purpose
⦁ Nebulizers: wet nebulizers are nebulized medications. Used for pts who cannot use an MDI inhaler properly or prefer this method of administration.
⦁ Optimal technique: slow tidal breathing with occasional deep breaths. Tightly fitting facemask for those unable to use mouthpiece.
⦁ Small-Volume Nebulizer (Mini-Nebulizer): handheld apparatus that disperses a moisturizing agent or medication, such as a bronchodilator or mucolytic agent, into microscopic particles and delivers it to the lungs as the pt inhales.
⦁ Indications: difficulty clearing respiratory secretions, reduced vital capacity, with ineffective deep breathing and coughing, and unsuccessful trials of simpler and less costly methods for clearing secretions.
⦁ MDI: metered dose inhaler. Pressurized device that contains an aerosolized powder of medication. A precise amount is released with activation. A spacer may be used to enhance deposition of med to the lung and help pt coordinate activation with inspiration.
⦁ To administer medication:
⦁ Remove cap and hold inhaler upright.
⦁ Shake the inhaler.
⦁ Tilt head back slightly and breathe out slowly and all the way.
⦁ Position inhaler approximately 1-2 inches away from the open mouth, or use a spacer/holding chamber. When using a medicine chamber, place the lips around the mouthpiece.
⦁ Start breathing in slowly through your mouth, and press down on the inhaler one time. If using a chamber, first press down on the inhaler and within 5 seconds begin to breathe in slowly.
⦁ Breathe in slowly and deeply for as long as possible.
⦁ Hold your breath as you count to 10 slowly (10 sec.) to allow medication to reach down into your airways.
⦁ Repeat puffs as directed, allowing 15-30 secs. between puffs for quick-acting medications. No need to wait for other types of medications.
⦁ Apply the cap to the MDI for storage.
⦁ After inhalation, rinse mouth with water when using a corticosteroid-containing MDI.
