Family Health Assessment Part II Local, national and international agencies are working together to decrease disparities related to health. Health promotion and the prevention of disease have taken... precedent in the United States in an effort to decrease chronic health conditions and promote healthy living (Falkner, 2018). Many factors that affect an individual's health is beyond their control. These factors typically are not directly influenced by the individual's choices, but rather stem from circumstances, such as poverty level, ethnicity or physical environment (World Health Organization [WHO], n.d.b). To assess the health of the Gayle’s a blended family in Covington, Georgia the Gordon functional health pattern assessment was used. By imploring this tool, the author was able to gather data to assist in discovering areas of health deficiencies. In this paper, the author will discuss the Gayle's family social determinants of health, recommend age-appropriate health screenings, create and outline a plan of health to promote family-centered health promotion. The social determinant of health refers to circumstances or conditions in which people are born, grow, live, play, work, and age. (Bryant, Hass, Bowen, 2015, p.22). After analyzing the data gathered from the family health assessment of the Gayle’s family. The author was able to identify a social determinant of obesity related to food dessert. Their diet consists of foods high in fat, sugar, and limited vegetables. Moreover, the Gayle's live in a rural community in Covington, Georgia. The nearest grocery store is ten miles away from their home. However, there are several fast food restaurants located within a half mile of their home. According to the Center for Disease Control (2017), rural, minority and low-income areas are often the sites of food deserts because they lack large, retail food markets and have a higher number of convenience stores, where healthy foods or less available. The social determinant of health of food deserts is impactful on the Gayle's. The Father reports his knees and legs ache when he stands for long periods of time. The mother is a borderline diabetic. She experiences blurred vision, fatigue and knee pain. Lastly, the 12- year old daughter suffers from low self-esteem due to obesity. She stated, she wears oversized clothes to hide her stomach.
