ATI MEDICAL SURGICAL NOTES Metoclopramide  Dopamine antagonist  BRAND NAME: Reglan  used short term to treat heartburn caused by gastroesophageal reflux  Gut utility stimulator  C... an also treat gastroparesis in patients w/ diabetes  Commonly used to treat and prevent nausea and vomiting  Increases muscle contractions in the upper GI tract  May develop a muscle problem called tardive dyskinesia, you will move muscles especially in the face in unusual ways  Take 30 mins BEFORE eating and at bedtime  SE: feeling restless, drowsy, tired dizzy, ha, insomnia, nausea/vomiting, diarrhea, breast tenderness/swelling, change in menses, urinating more than usual To alleviate shock: administer O2, apply pressure to any obvious bleeding, elevate feet to shunt blood to vital organs Glasgow Coma scale- low score 3 totally unresponsive High score 15 client within normal limits ABCDE principle: Airway/cervical spine- MOST IMPORTANT, if client responsive & awake, AIRWAY IS OPEN, if AIRWAY LOST, then check for blood, broken teeth, vomitus or other foreign materials in the airway If client unresponsive w/o suspicion of trauma, head tilt/chin lift maneuver BUT DO NOT PERFORM THIS ON CLIENT W/ POTENTIAL CERVICAL INJURY IF UNRESPONSIVE W/ SUSPICION OF HEAD/NECK/SPINAL CORD TRAUMA the-n use MODIFIED JAW THRUST MANEUVER BAG VALVE MASK w/ 100% oxygen for additional support during resuscitation NONREBREATHER MASK w/ 100% oxygen for those spontaneously breathing Epinephrine- vasoconstriction, increases heart rate, contractility, conduction through AV node, bronchodilation.
