Comprehensive Client Assessment /
Psychiatric mental health nurse practitioners (PMHNPs) provide care to clients with
mental health disorders and substance use disorders. Substance dependence is one of the major
chronic disorder with high mortality and morbidity in the today’s world, with a relapsing and
remitting course (Choudhary et al, 2017) . To best treat clients with substance issues Initial
assessment must be done fully and completely to understand when and why. Ideally, information
needs to be collected continuously, and assessments revised and monitored as the client moves
through recovery. A comprehensive assessment leads to improved treatment planning, and it is
the intent to provide a model of optimal process of evaluation for clients with substance issues
and to encourage the client to participate in in recovery(Adam et al. 2004) .
Demographic Information
The client is a 16-year-old Caucasian male who speaks English and who resides with his
father and his two older brothers in a town house in downtown phoenix area. The client does not
have any children of his own. The client does not acknowledge and religious belifes currently.
The client identifies as a straight male and denies being sexually active currently. The client is
covered, under his parents’ health insurance united health insurance community plan Arizona.
The client’s father works long hours for a landscaping company in the phoenix metro area and
His mother works as housekeeper at a large hotel chain in the Phoenix area. Their household
income puts them in lower economic standing. Associations between individual-factors shown to
have a statistically significant independent association with worse mental health were low
income, lack of social support, low level of education, low income, low socioeconomic status
( Fussell, E., & Lowe, S. R. 2014). Client was attending school at Global academy before
getting in trouble for substance use and possession. His older brother is 18 years old but did not

graduate high school and has a history of substance abuse and probation for illegal substance.
Client had an older sister that would be 22 years old but died from accidental overdose one year
ago. Client arrived at the psychiatric in-patient treatment center from Durango jail where he is on
probation for possession and substance use. Client most complete in-patient treatment for his
probation
