Introduction of Transitional Care Model to Meet Patient Safety Goals Nursing care delivery models are a key concept in meeting patient safety goals in the healthcare setting as well as providing high quality care continually. This organization currently uses total-patient-care and management has found that it is not working for this organization in order to meet the joint commissions challenges. The proposal of introducing a new patient care delivery model of transitional care will help to reduce facility costs and improve patient safety goal initiatives to meet the Joint Commission challenges that we were presented. The two patient safety goal initiatives that we will work on will be to improve staff communication and prevent readmissions [The18]. Transitional Care Delivery Model and Patient Safety Goals The transitional care model is a type of nursing care delivery system that has an advanced nurse leading a team-based care management approach to patient care[Nay18]. Staff communication is a key component to making sure that patients receive the help that they need in a timely manner, especially when it comes to important test results. Transitional care involves a broad range of practices that help nurses to deliver quality care to patients from their hospital stay and continuing to rehab or homecare. The interventions that are used help with the coordination of care for the patient[Fid17]. By using a transitional care model, a single point person becomes the patients’ liaison. Improving communication can also be a key component in preventing infections. The patients’ liaison can ensure that the patient has a continuity of care upon their release from the hospital. “The TCM emphasizes identification of patients' health goals; design and implementation of a streamlined plan of care; and continuity of care across settings and between providers throughout episodes of acute illness” (Hirschman, Shaid, McCauley, Pauly, & Naylor, 2015).
