CHF and how this affects their compliance with disease management. Looking at this subject from an ambulatory care point of view gives a better understanding if policies that are in place to contact patients by telephone within 48 hours of discharge and then again at sequential intervals is actually making a difference in these critical patients disease management, or if face-to-face appointments with the doctors are a better treatment course. The purpose in this meta-analysis of these three studies is to determine if re-admissions in COPD/CHF patients are lessened with frequent telehealth follow-ups by nursing. Clinical Question The clinical question that was researched was in hospital discharge patients with COPD or/and CHF (P), how does nurse tele-health follow-up (I) compared to no tele-health follow-up (C) affect hospital disease management within six months to three years? Justification of the Topic The need for this topic to be investigated was implemented to evaluate if COPD and/or CHF patients were receiving adequate b follow up with good medical evaluations and education over the phone as opposed to having them attend face-to-face in clinic appointments changing the outcome of their personal disease management control.
