Lab: ATI-Real Life 3.0 GI Bleed 1 Firstly, there were many problems and issues that occurred in the GI bleed ATI scenario with the patient. One of the problems that the patient Ms. Lieberman had a... t the beginning of the scenario was that she stated she felt lightheaded and dizzy, which showed possible signs of hypovolemic shock due to her blood loss. The next serious issue that occurred was that the patient reacted to the blood transfusion, causing them to stop the transfusion. The patient also states she is very stressed and sometimes has several alcohol drinks a night, which is a big problem and can affect the patients’ physical and mental health. Secondly, I think an aha moment for me was when the charge nurse came in and asked if the patient was cold, and the nurse stated yes. To my way of thinking, I would have started checking for other signs of a transfusion reaction as soon as the patient said she was cold. Sudden onset of chills is a sign of a transfusion reaction. Another aha moment was that the nurse did not change out the line and restart the normal saline.
