Case Study 2: Patient HM has a history of atrial fibrillation and a transient ischemic attack (TIA). The patient has been diagnosed with type 2 diabetes, hypertension, hyperlipidemia and ischemic he... art disease. Drugs currently prescribed include the following:  Warfarin 5 mg daily MWF and 2.5 mg daily T, TH, Sat, Sun  Aspirin 81 mg daily  Metformin 1000 mg po bid  Glyburide 10 mg bid  Atenolol 100 mg po daily  Motrin 200 mg 1–3 tablets every 6 hours as needed for pain About three out of four people who have a stroke for the first time have high blood pressure. Approximately 33% of adults in America are affected by hypertension sometimes called “high blood pressure” (Arcangelo, Peterson, Wilbur, & Reinhold, 2017). According to National Stroke Association (2018), High blood pressure, atrial fibrillation (AFib), high cholesterol, diabetes and circulation problems are all medical risk factors, which are controllable, for stroke. Age: Pharmacokinetic and Pharmacodynamics Process Age should be taken into consideration when providers prescribe medications for the elderly population. Scondotto et al., (2018) study show there are various homeostatic changes that occur in the human body such as a reduction in the homeostatic ability as the individual’s age increases. The increasing age affects the renal system of the individual. Reduction in nephrons and hyalinization leads to reduced blood flow in the body (Scondotto et al., 2018). Adverse drug reactions increases in pharmacodynamics and a decrease of pharmacokinetic with an elderly population (Arcangelo et al., 2017). Pharmacokinetic changes include a reduction in renal and hepatic clearance and an increase in the volume of distribution of lipid soluble drugs (hence prolongation of elimination half-life) whereas pharmacodynamics changes involve altered (usually increased) sensitivity to several classes of drugs such as anticoagulants, cardiovascular and psychotropic drugs (Mangoni & Jackson, 2004).
