Purpose Capstone Portfolio Guidelines with Scoring Rubric The purpose of the Professional Portfolio is to demonstrate your professional growth and expertise by providing evidence of the knowledge you have learned. The Professional Portfolio serves the following purposes: Promotes the synthesis of knowledge – To become a competent leader in the nursing profession and a selected specialization, you need to synthesize a great deal of information from a variety of sources. Promotes reflective thought – One hallmark of graduate education is the ability to think analytically and engage in self-assessment. Creating the portfolio allows you to evaluate and demonstrate your achievements in acquiring advanced nursing and specialty knowledge. Provides evidence that assignment, course, and program learning outcomes have been met –The portfolio reflects the different ways you have met the learning outcomes. Directions – This assignment should be an entire document in either word or PDF format with a title page noting the assignment name, student name, course, etc. The assignment should also include a table of contents denoting what of the below documents are on which pages. Each section should be its own page. Due Date: Saturday 11:59 p.m. (MT) at the end of Week 8 Total Points Possible: 100 Portfolio Evaluation Methods copy of the job posting. Curriculum Vitae (see CV template) – All components present as noted on the template 15 Salary expectations and justification 5 Malpractice insurance ( Copy of a Malpractice plan and cost for FNP) 5 eLog summary of patient experiences (demographics, ICD-10 and CPT) For this course and one for entire clinical courses 15 Letters of reference or recommendation (If available) * Awards, honors, or distinctions * Presentations, articles, or reports (SOAP notes x 5 and Reflections x3 be sure all corrections from previous grading have been made) * Description of special projects * Certificate of completion for all of the APEA learning modules in the NR 661 course 10 Copies of or Screen shots (*all below) Board of nursing application APRN (screen shot or PDF) 2 Certification application (American Nurses Credentialing Center or American Association of Nurse Practitioners) 2 Drug Enforcement Administration application (screen shot) 2 State prescribing and dispensing application (if applicable, of not – include statement of why) 2 National Provider Identification (NPI) number application screen shot) 2 Copy of National competencies for family nurse practitioners 5 Content Subtotal 100
