Assessment Social environment health domain focuses on assess social isolation in the geriatric population. Social isolation is prevalent in the older population but sometimes is not address in the primary care setting (Kennedy- Malone & Martin-Plank, 2109). Social isolation is associated with mortality and mobility. The geriatric population suffers “Shrinking social network due to deaths of one’s peers” (Seematter-Bagnoud & Büla, 2018, p. 9), which means a lot of peers and friends are dying, so they isolate themselves. It is essential to assess social support. What is their social support, and who is the person that contact. The following concepts are address as parts of the social environment, such as Living and housing conditions, transportation, income, assets, and financial status (Kennedy- Malone & Martin-Plank, 2019). Quality of life measure is another domain that focuses on assessing the person’s general satisfaction with life and evaluate their functional conditions. In this domain, the end of life preference is addressed; patients are encouraged to complete a document stating their end of life wishes.

The assessment tool has three items that help to understand patients' social support. The three elements include having a relative or friend that can reach out when they need to speak to someone about the personal matter (Evans, Llewellyn, Matthews, Woodds, Brayne, & Clare, 2018). The responses are obtained utilizing six category responses. The number of relative or friends see or heart from a least once a month from 0= one to 5= nine or more. The score is based on the sum of the six questions. The higher rating means that more social supports, and the score range between 0-30 (Evans et al., 2018).

Preferences for care Medical outcome scale Short Form -36 is an assessment tool that is utilized to evaluate vitality, physical function, body pain, general health, physical role, emotional role, social role, and mental health (Laucis, Hays, & Bhattacharyya, 2015). The patients have to answer the question based on how they felt the week before the visit and how well they can do their activities (Kennedy- Malone & Martin-Plank, 2019). The score is the sum of
each questions’ responses to the evaluation. “The higher represent better health” (Laucis et al., 2015, p. 1629). The score range from 1-100. This test is not designed for older adults but having use as part of the CGA.
