Synopsis:
Type 2 diabetes is a chronic disease characterized by high levels of sugar in the blood.
The food we consume is broken down into basic components during digestion with
carbohydrates being broken down into simple sugars such as glucose. Glucose is a critical
component that provides an important source of energy for the body's cells by leaving the
bloodstream and getting inside the cells. The body allows glucose to merge into the cells for
energy via insulin traveling in the bloodstream. It senses that glucose is available for use and
signals the cells to allow entrance. When we eat, levels of glucose keep rising in the bloodstream
jolting the pancreas to produce more insulin. When the human body produces too much insulin
over a long period, our body's cells start developing resistance to the normal effect of insulin
causing what we know as Diabetes Mellitus type 2. As a result, glucose starts building up in the
blood making the Pancreas work overtime however, eventually it gets tired from overworking
and cannot keep up with the demand for more insulin creating full resistance.
Folks who are at an increased risk for developing type 2 diabetes can take steps to prevent type 2
diabetes by maintaining your ideal body weight, exercising regularly (regular exercise reduces
insulin resistance), eating a healthy diet low in sugar and processed foods and by being
compliant with medications.
The shadow health antidiabetic concept lab was very unique as it did not take us into a
clinical office visit but instead, creating an interactive learning environment. We learned about
various oral antidiabetics used to treat Diabetes Mellitus type 2 through animations, medication
summaries and a quiz at the end to test the student’s knowledge. These medication summaries

discussed various therapies in the treatment of DM2 including examples of drugs in each class,
their mechanism of action and the most common or serious side effects. The animations were
very expressive allowing me to see how the drug works in the body/bloodstream. The course
went over the following medication classes:
Biguanides
GLP – 1 agonists
DPP – 4 inhibitors
SGLT – 2 inhibitors
Thiazolidinediones
Alpha – glucosidase inhibitors
Sulfonylureas
Meglitinide analogues
Overall, the course was very informative although, in my personal opinion, the alternate
method of learning in shadow health is more effective in learning about medications in general.
Being able to apply knowledge in the clinical setting allows me to make connections with the
theory learned in class and remember all the considerations to be mindful of when prescribing
anti-diabetic medications.
What did you learn?
Although there was such a variety of topics and drugs to learn, I had a special interest in
Metformin being that I preop and recover patients for the Cardiac Cath Lab. Diabetic patients
often have heart disease which is usually exacerbated by poor glucose control in patients lacking

compliance. I see Metformin often when I reconcile medications at work and it is important to be
diligent when questioning patients about the use, especially when going into a procedure.
Patients in the Cardiac Cath Lab receive a considerable amount of contrast, which is needed to
confirm guidewire placement in the vessels as it is floated up into the coronaries. The lecture on
shadow health, as well as the lecture posted on canvas, emphasizes the risk for renal disease with
patients on Metformin. Pre-cardiac cath lab patients are always instructed to stop the medication
two days prior due to the risk of acute renal failure from too much contrast mixed with
Metformin in the bloodstream and often, we have to pre-hydrate patients if they do not follow
instructions well and creatinine clearance is high on the day of the exam. It was also very
interesting to learn that Metformin is a preferred drug due to its high efficacy, low risk of
hypoglycemia and low-cost making is easily accessible to a large number of Americans, even
those without health insurance.
