The first cranial nerve, the olfactory nerve, has only sensory function. It is a tiny nerve that runs along an irregular bony surface of the skull and is quite easily disrupted with head injury. Test ... ing this nerve, although often omitted, requires having an aromatic, non-noxious scent available. Mint, spices, and coffee will all work but are not always readily available. Toothpaste is often available and may work, but avoid the temptation to use an alcohol wipe. Noxious odors such as alcohol may stimulate the trigeminal nerve and cause a misleading response. To test the first cranial nerve, simply have the patient close his eyes, occlude one nostril, and identify the scent you offer. Repeat on the other side. Cranial nerve II, the optic nerve, is a sensory nerve that is best tested by checking visual acuity. Distance vision, tested using a Snellen chart the patient reads from a distance of 20 feet, is rarely part of a general physical exam. Testing near vision, however, is easy to do in most practice settings. [Show More]
