Poorly controlled diabetes negatively affects fetal oxygenation throughout pregnancy. In utero,
erythropoiesis accelerates to meet additional fetal oxygen needs, which creates RBC needed to
transport oxygen to tissue. An infant born to a diabetic mother will have polycythemia
(hematocrit >65%).
For infants, use of a 1-inch (25 mm) needle to administer medication in the vastus lateralis
muscle is recommended to reach the IM tissue and minimize local reactions.
During dressing changes, injection caps, and tubing changes place the client in a supine
position. If A CVC causes an air embolism, place them in Trendelenburg on the left side, so that
existing air can rise and become trapped in the right atrium.
Have client turn away from catheter site and suing cap/tubing changes have them perform
Valsalva to prevent air from entering the line, traveling to the heart, and forming an air
embolism.
For positional plagiocephaly:
- Minimize time in car seat
- Prone only when awake for 3-60 minutes a day (tummy time)
- Place favorite things opposite affected side to encourage turning head
- Alternate the infants supine head position from side to side
When an electronic assessment is questionable the nurse should always assess the client
(erratic plethysmograph waveform)
A higher bishop score indicates the likelihood of successful induction that results in vaginal birth.
For women with no children, a score of 6-8 or greater is good.
Symptoms of a ruptured ectopic pregnancy include hypotension, tachycardia, dizziness, and
referred shoulder pain.
Tension pneumothorax:
- Tracheal deviation
- Hypotension’
- Perform large bore needle decompression
With a hiatal hernia: occurred due to weakened diagram and increased intraabdominal pressure
Nursing intervention like Gerd (want to decrease pressure)
- Avoid high fat foods and things like chocolate, Pierpont, tomatoes, caffeine
- Small frequent meal, no drinking wine eating
- Elite heard 30 degrees with 4-6-inch blocks
- No girdle because increases pressure
