Summary, NURSING PNP401: Weeks 6 & 7 Healing Practices Reviews -Class Notes Part A : Use DOBI SPARK good for this lesson covers; ● Malnutrition→ who’s most likely to be, how we treat it, possi... bility of using TPN, ● Dysphagia ● nausea/vomiting→ antiemetics, ect… ● GERD GASTRITIS Most likely to cause it: NSAIDs & ETOH (more so with binge drinking acutely) → it’s inflammation of the gastric mucosa - b/c something has irritated it MANIFESTATIONS: Acute - N/V - Anorexia → b/c the pain/irritation makes you not want to eat - Epigastric tenderness (gut rot feeling) - Feeling of fullness - Hemorrhage → this is a single symptom with ETOH abuse or chronic use of NSAIDs Chronic - Vague complains as above + → Pain worsening with intake of spicy food or fatty foods → Epigastric pain may be relieved with food - Anemia → cobalamin deficiency (not enough vitamin B12) therefore give them vitamin B12 replacement therapy - Weight loss DIAGNOSTICS: ● With pt. Hx ○ How long has N/V going on for (N/V won’t be chronic for gastritis) ○ What’s your NSAIDs & ETOH intake ■ Being specific … NSAIDs… do you take medications like; advil, ibuprofen, naproxen, aspirin on a regular basis & what’s your regular basis ● Testing for H-pylori ○ Many have it but often lies dormant ■ Tests pt. Breaths into a bag drink a special beverage and exhale into another bag the drink they have has carbon molecules if they have h-pylori break down the carbon molecules and CO2 breathing out is higher *she’s says not to know* → many other tests as well ● CBC ○ Hbg → looking for anemia ● Stool for occult blood (FOBT) ○ Only if you can’t see the blood in the vomitus or stool ● Endoscopy ○ Not the first line due to it being invasive used lastly ○ If we do do an endoscopy often for those acutely bleeding (like seeing blood (frank bleeding) in the vomitus) PHARMACOLOGY: NSAIDs - Find new medication OR have the pt. Take with food Helpful Comfort Medication Treatments *helps Gastric but also helps for GERD* CYTOPROTECTIVE (over the counter) ⇒ protects the mucosa ● Acid-Protective (sucralfate) ● Alginic acid-antacid (Gaviscon) ANTI-SECRETORY (prescription) [Show More]
