Healthcare Value and Ethics-Case: Jamilah Shah A1. Potential Ethical Dilemmas Jamilah Shah was brought to the hospital after an incident after she collapsed. After arriving at the hospital tests wer... e completed to determine a diagnosis. Upon completion of medical tests, it was determined that Jamilah suffered a heart attack. Now that the medical staff knows the cause of her condition, decisions need to be made concerning further care. Because of the difference of medical opinions, the opinions of certain family members, and the statement of the social worker that spoke with Jamilah on a previous occasion regarding her wishes to be saved, there can be a plethora of different ethical dilemmas. Three specific examples of ethical dilemmas can be seen with Autonomy, Beneficence, and Nonmaleficence. A1a. Ethical Principles and Legal Requirements Autonomy: Jamilah resides in an extended care facility it could be assumed that she cannot maker her own decisions. Jamilah has no advance directive and comes from cultural beliefs and a family structure where males take control, provide and make decisions for family members. Because of these cultural beliefs her sons, particularly Bashir, has taken the responsibility of making her decisions. In another situation, Jamilah had expressed to her social worker that she did want help and wanted to take measures to save her live when the opportunity presented itself. Because of all these factors it is uncertain whether or not Jamilah is capable of making her own decisions. Legal implications: Because Jamilah has the right to her autonomy and she is awake, physicians and medical staff have the duty to inform her of the risks, benefits, and alternatives of the treatment they want to give her. Even though she only understands basic English, and the social worker is the only one who is voicing her wishes to live, Jamilah could find a way to reach out to someone so that she could fight for the treatment she wants.
